1. Complete this form and return it through regular mail or in person to the office at Box 40, Loreburn SK, S0H 2S0, or email to office@rm254.ca
2. Applications returned by post or email must be accompanied with a copy of identification documents, to establish identity
   and residence (see below). DO NOT SEND ORIGINALS.
3. If approved, applicants will be sent a mail-in ballot kit. Kits will be mailed approximately [Date]

COPY OF ID* You must show identification that proves your identity and residence.
Option 1: Attach a copy of your government-issued photo ID to this application. This can be a
Saskatchewan Driver’s license or a Saskatchewan non-driver photo ID.
Option 2: Attach a copy of two pieces of information or documents. Both pieces must show your
